Mailing your Letter of Assurance and Budget Forms
The PA Cares Application is an online process and must be completed and submitted online. No other form of the application will be accepted. The Letter of Assurance (LOA) and Budget Forms should be mailed with a postmark no later than November 13, 2009 to:
